Sense Health, a New York-based mobile health technology company, announced its partnership with Coordinated Behavioral Care Inc. (CBC)’s Health Home, which provides care coordination to Medicaid beneficiaries with serious behavioral and/or medical conditions. CBC is deploying Sense Health with 12 of its NYC care management agencies. Care coordinators will use the platform for two-way text message communication, including motivational, and reminder messages to increase patient engagement with their treatment services, such as adherence to scheduled appointments and prescribed medications.
This partnership comes at a pivotal time as New York State (NYS), the country’s second-largest state Medicaid program, undergoes a $6.42 billion delivery system redesign (DSRIP). The overall goal of DSRIP is to reduce avoidable hospital use by 25%.
