In this first session, we will focus on two tactical areas. First, how DSRIP PPS leaders can analyze participating provider data to facilitate project planning, outreach, and program success. Second, an industry expert from Primary Care Development Corp will provide a helpful overview of how organizations can prepare for and achieve Patient Centered Medical Home (PCMH) recognition. There will be more sessions to follow and we welcome your input to help shape future content to assist those working to transform healthcare in New York State.
