Hi, we’re NYeC, the New York eHealth Collaborative. We help lead New York’s Health Information Exchange Network, The SHIN-NY, which connects healthcare providers across the state so that they can coordinate and collaborate to improve patient care. The SHIN-NY helps providers care for millions of New Yorkers each year. In these incredibly difficult times, every bit of help counts. What does this help really look like? Let’s see an example.
This is Anna. Like so many of us, Anna has multiple chronic diseases. In addition to visiting a local primary care provider, Anna also sees specialists outside of her region. Using The SHIN-NY, Anna’s care team can easily share the latest information about her condition, access test results, and coordinate to improve her care and make it more efficient by reducing unnecessary tests and appointments. This is possible because Anna’s providers share and view their patients’ records using one of six regional health information networks. The SHIN-NY connects and securely links regional networks to create a statewide resource for coordination and collaboration.
Currently, 100% of the hospitals in the state use The SHIN-NY, and over 100,000 care providers, accessing it directly or via their electronic health record systems, sharing information like clinical history, medication information, lab results, visit information, and more.
Let’s get back to Anna. One day, Anna urgently visits the hospital. Thanks to The SHIN-NY, Anna’s ER physician can see any records from her other providers that might impact her care, including life-threatening allergies, critical medication interactions, and information about her other conditions. The collaboration doesn’t stop there, though. Through The SHIN-NY, the rest of Anna’s care team gets alerted as soon as Anna is admitted to the ER. When Anna leaves the hospital, her providers know what happened and can access her new test results and treatment plans, so that when she gets home, they’re already prepared to help.
Use of The SHIN-NY is associated with a 26% reduction in the rate of emergency department visits, a 50% reduction in the rate of hospital readmissions, and annual healthcare system savings of approximately $160 to $195 million. Connecting providers as they collaborate to deliver coordinated care and improved patient health, it’s what The SHIN-NY does to help healthcare providers every day. Because when providers get the information they need, patients get the care they need, and that’s what matters most.
